Passports

Passports applications are accepted in the Office of Clerk of the District Court. Appointments are not required but a phone call is suggested over the lunch hour.

New Applicants will need:

  • DS-11 application form (available in the office or on the U S Department of State website) – https://travel.state.gov/content/travel.html
  • Certified state issued birth certificate (or naturalization papers)
  • One passport photo (photography services not available in the office)
  • Drivers license or picture id.
  • Payment may be made by personal check or money order. Separate payments are needed for the passport fee and for the execution fee. Credit cards are not accepted.

Passport renewals in many instances can be done by the applicant using form DS-82 available on – https://travel.state.gov/content/travel.html

Passport Photo Requirements

  • One passport photo is required
  • Must meet U.S. Department of State size and specifications

Proof of Citizenship (Provide ONE)

  • Birth Certificate
  • Naturalization Certificate
  • Previous Adult Passport

Proof of Identity

  • Age 16 and older: Driver’s License
  • Under 16: Parent(s)’ Driver’s License or Identification Card

Costs

  • Adult Passport: $165.00
  • Youth Passport: $135.00
  • Expedited Service: $60.00 per passport
  • Overnight Express: Contact the office for current postal rates

Processing Time

  • Call the office for current processing times, especially for expedited applications.

Renewals

  • Applications available at the Clerk of District Court Office
  • Or online via the U.S. Department of State

Special Requirements for Youth (Under Age 16)

  • Both parents must sign the application
